Insurance and Health Plan Info.
We work with most insurances & accept most plans within their networks. Please check with your insurance prior to your appointment to confirm your benefits and any specifics regarding your care. Verification of benefits is not a guarantee of payment, and you are ultimately responsible for payment of all services that are not covered by your insurance plan once the claim has been processed. All copays, deductible payments, and coinsurance payments are due at the time of service.

Please bring your insurance card and a photo ID to your visit. It is very important for you to bring your current insurance card to each visit and inform us if there have been any changes in your coverage or in your address and phone number.NOTE: Changes may occur in the plans that we accept; please confirm with your insurance company at the time you make your appointment.
Patients Without Health Insurance Coverage:
We understand that not everybody can feasibly afford health insurance coverage, and in response, Scott Medical Solutions has set up the following pricing for self-pay visits.
- Initial Visit/Establishment of Care: $135.00
- Follow-Up/General Office Visit: $80.00
